MAIDSTONE, Sask. — Police in Saskatchewan say a man was found gravely injured and died several hours after a call for a wellness check that wasn't done.

Maidstone RCMP say they received the request for a wellness check on the man shortly after 7 a.m. Wednesday, but officers did not physically confirm his well-being.

Ten hours later, Mounties received a report of an injured man at a home in the Rural Municipality of Britannia, near the Alberta boundary, and he was taken to hospital, where he died.

The Serious Incident Response Team, the province's police watchdog, is investigating the response to the initial call for a wellness check.

Mounties say they're doing their own internal probe to determine if officers followed policy.

The RCMP's major crimes unit, forensic investigators and the coroner are investigating the circumstances around the man's death.
