This unique model of care is the reason Dr. Amy Weber joined Copeman seven years ago. “When I first looked at Copeman, it was exactly how I envisioned practicing medicine,” says Dr. Weber, Medical Director and a Family Physician at Copeman.

“Clients benefit from having a multi-disciplinary primary care team that offers diverse but aligned care for their overall health. It is family medicine teamed with nurse practitioners, RNs, kinesiologists and dietitians. It’s about having a relationship not only with your doctor but also a broader healthcare team who are engaged and passionate about optimizing your health.”

For instance, if a new patient comes in with elevated blood glucose and is diagnosed with diabetes or pre-diabetes, the doctor will work with the whole care team, including a kinesiologist, family health nurse and registered dietitian certified as a diabetes educator, to create a personalized and proactive care plan.

“You get the entire team involved in supporting and educating that patient,” says Dr. Weber.

This collaborative, chart-sharing approach includes nutrition counselling, exercise medicine and personal training, as well as access to genetic testing through the Precision Health program, physiotherapy, and mental and cognitive health programs.
